can the icons created with realworld be copied to an NT system to say the system32 folder and then used tagged to a program in the usual manner
Yes, but you should delete (or not generate) the images from the icon, which are not used on NT. Basically, only leave image of 16x16 and 32x32 pixels in 4-bit and 8-bit color depths.

Yes, choose to create a custom icon and uncheck the superfluous formats and color depths.
